The Universal Care Plan is an online document that captures your preferences and wishes regarding your care, such as your preferred location for treatment in case of sudden illness. It also allows for updates on individuals involved in your care, including relatives and your next of kin.
One of the key benefits of this plan is that healthcare professionals can access it quickly and easily. This means that paramedics, the 111 service, and out-of-hours doctors will have immediate access to your care preferences and important information.
