In PCN3 we have a large and well established team of clinical pharmacists. Our clinical pharmacists have expert knowledge of medicines for particular diseases. They work with the healthcare teams in our GP surgeries to help patients with long-term conditions.
Clinical pharmacists carry out medication reviews for patients taking many different medicines. These include elderly patients, patients in care homes and patients with more than 1 condition.
